Dr. Schmugler has retired!

Practice Closure June 25, 2026.

From myself and all of our staff, a heartfelt thank you to each and every patient who has been a part of our practice.

I regret that I am unable to have a longer transition period, but I had sudden notice that our office space is no longer available. It is with a heavy heart that as I near retirement age, I don’t want to take on the challenge of opening a new office. I’m thankful for all the time we’ve spent and getting to know you all. How to Obtain Your Medical Records

Although the practice is now closed, your medical records are being securely retained, and you have the right to request a copy of them at any time.

To protect your privacy, all records requests are handled through a secure, encrypted online contact form. Please click the button below to complete your request:

If you have any issues or trouble with the form, please email us at footworks@footworkspodiatry.com and we will help you complete your request. Please do not include confidential or protected health information in your email.

The form will ask you to provide:

  • Your full name and date of birth
  • Your current contact information (address, phone, and email)
  • Proof of identity
  • The dates or type of records you are requesting, if known
  • Where you would like the records sent (to you, or to another provider)

If you are requesting records on behalf of someone else — for example, a minor child or a patient for whom you are the legal representative — please indicate this on the form. We may ask for documentation confirming your authority before releasing records.

What to expect

We will verify your identity and process your request as promptly as possible, and in all cases within 30 days as required by law. Records are delivered securely and electronically whenever possible. A reasonable, cost-based fee may apply for certain formats (such as printed or mailed paper copies); if any fee applies, we will let you know before processing your request.
